Why this app important:
Vascular access is the lifeline of these patients and for the dialysis to be effective, the vascular access must be able to handle adequate blood flow to the dialysis machine for purification and return to the patients. Unfortunately, all types of vascular accesses tend to fail over time and maintenance is required to keep them open with adequate blood flow for good clearance.
The current referral process for access problems is time consuming. The referral often requires the nurse, nurse practitioner, physician assistant or nephrologist to fill out long forms and then fax the forms to the vascular access center. This is often followed by a call to the access center to confirm receipt and schedule the patient. This workflow consumes tremendous amounts of time and effort and hence many patients do not get timely referral resulting in failed accesses which now require emergency room visits and more difficult procedures.
To alleviate the burden of this workflow, Flatlands has re-created the workflow through this app, to make it quick and easy to refer a patient. In addition, once a user has customized a list of patients, with the click of a button, a referral and confirmation can be done, and on the go.
User registration:
Authorized users are nurses, advanced practitioners and doctors. Registration is simple and once logged in, user can activate finger/face authentication for future ease of access. User will then have access to instructions for use
