Our mission is to help nephrologists and dialysis centers meet their CMS quality of care metrics, reduce unnecessary costs related to emergency room visits and admissions, reduce missed dialysis sessions, and reduce catheter dialysis centers.

We will achieve this mission by providing:

Interventional nephrology vascular access procedure
  1. Seamless Access to Maintenance Services. With the same day appointments for emergencies such as clotted accesses and malfunctioning catheters, we aim to resolve emergency issues without delay and resolve all issues for scheduled patients timely.
  2. Seamless Integration. We see ourselves as a resource in the continuum of care, to make the dialysis unit successful through the ACCESS STRATEGY below.
  3. Aggressive Vein Mapping & Creation. We are a multidisciplinary team of interventional nephrologists and surgeons working together every day to ensure that we prioritize vein mapping, selection of suitable veins if any and timely creation of new fistulas/grafts. By reducing the catheter rates, the CMS quality metrics will improve, translating into improved revenue for dialysis centers.

The Flatlands Rule of 4 Strategy for New Accesses

1Vein mapping done before dialysis or within four (4) weeks of starting dialysis
2Fistula placement within four (4) weeks of vein mapping
3Assessment for maturation at four (4) weeks
4Fistula usage and catheter removal within four (4) weeks of maturation
